29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a) On or about 07/24/17, on the front side of the roof on a house at 61 Tulane Rd., Kenmore, NY: Two employees engaged in roofing work on a steep roof were exposed to a fall of approx. 18 foot. No fall protection system was being utilized. Pitch of the roof was approx. 8 in 12. NO ABATEMENT CERTIFICATION REQUIRED Ivy Lea Construction Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 947770, citation number 1, item number 1a and was affirmed as a final order on 12/12/2013, with respect to a workplace located at 293 Colvin Avenue, Buffalo, NY 14216.

29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system: a) On or about 07/24/17, on the front side of the house at 61 Tulane Rd., Kenmore, NY: Two employees were standing, walking, and working from the approx. 14 inch wide aluminum "pic" of a ladder jack scaffold. The employees were exposed to a fall of approx. 16 ft. to the ground. No fall protection system was utilized. NO ABATEMENT DOCUMENTATION REQUIRED Ivy Lea Construction Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 947770, citation number 1, item number 1b and was affirmed as a final order on 12/12/2013, with respect to a workplace located at 293 Colvin Avenue, Buffalo, NY 14216.
