29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) On or about 8/1/2017, machine guarding was needed to protect operators from ingoing nip points on the DUSE #2. The nip point where the rolls form was approximately 16" in from front tray. b) On or about 8/1/2017, machine guarding was needed to prevent employees from reaching into the DUCE #3 pounder area.
