29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) On or about 11/19/2018, ETI-1 COHESIO machine had unguarded ingoing nip points formed by the rollers on Infeed #1 and Infeed #2. b) On or about 11/19/2018, ETI-1 silicone unit has rolls that need to move inside 8" x 16" opening. Out feed nip roller approximately 4" inside of frame of machine is not guarded. c) On or about 11/19/2018, ETI-1 COHESIO machine had unguarded slitter blades on the top and bottom units creating an amputation hazard. d) On or about 11/20/2018, Press #9 missing guards by turret exposing nip points during machine operation. e) On or about 11/20/2018, Press #10 GONDERFLEX, Had exposed ingoing nip points on rollers approximately 6" inside of frame at die station and perforation die. f) On or about 11/20/2018, Press #8 had exposed ingoing nip points on the rubber roll on top of the drive roll. The Heartland Label Printers LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.212(a)(1) which was contained in OSHA inspection number 1249787, citation number 01, item number 001 and was affirmed as a final order on 1/18/2018, with respect to a workplace located at 1700 Stephen Street, Little Chute, WI 54140.
