29 CFR 1910.22(a)(2): The floor of every workroom shall be maintained in a clean and, so far as possible, a dry condition. Where wet processes are used, drainage shall be maintained, and false floors, platforms, mats, or other dry standing places should be provided where practicable. Brewery: Drainage hoses and puddles of water had collected on the floor where employees enter. Drums, pails and stored equipment were placed on both sides of the floor creating a slipping and tripping hazard.

Employees were exposed to falls of twelve feet.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket): Housekeeping/Laundry Room: Four radio chargers were found plugged into an active power strip with nearby cardboard boxes. The area was used for storage and had not been disturbed for over a year. A fan was plugged into a power strip. The strip plug wire connection had damaged insulation.
