29 CFR 1910.22(a)(2): The floor of every workroom shall be maintained in a clean and, so far as possible, a dry condition. Where wet processes are used, drainage shall be maintained, and false floors, platforms, mats, or other dry standing places should be provided where practicable.  Brewery: Drainage hoses and puddles of water had collected on the floor where employees enter.  Drums, pails and stored equipment were placed on both sides of the floor creating a slipping and tripping hazard.

(i) Except as provided elsewhere in this section, the employer must ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the following:    Maintenance Shop: The upper platform on the mezzanine did not provided a guardrail system at the loading area and the top rail was at a height of approximately 25 inches with the chain sagging from the floor.  Employees were exposed to falls of twelve feet.

Loading Dock: Electrical box had a missing knockouts, it provides hallway power for the trash compactor; the main bay of the maintenance shop and the east of the walk-in freezer.  B. Upholstery Shop: Electrical box had a missing knockout, it provides power for exterior plugs, table saw light and dust collector.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket):  Housekeeping/Laundry Room:  Four radio chargers were found plugged into an active power strip with nearby cardboard boxes.  The area was used for storage and had not been disturbed for over a year.  A fan was plugged into a power strip.  The strip plug wire connection had damaged insulation.
