29 CFR 1910.134(c)(1): The respiratory protection program was not updated as necessary to reflect those changes in workplace conditions that affected respirator use:         a.)  Anchor an Oldcastle Company, Production areas, on or about July 27th, 2017 and times prior thereto:  Employees were required to wear filtering facepiece respirators (3M Brand #9211+ Aura particulate respirator N95) during work tasks including, but not limited to, bagging various silica based products.  The Company's written Respiratory Protection Program was not updated to indicate that respirators were required to be worn by employees and it did not include site specific information including, but not limited to, the following:       1.)  Identification of the current physician or other licensed health care professional that will conduct medical evaluations;   2.)  The specific type/style of respirators to be used at the worksite;   3.)  Updated information regarding the person(s) designated as the program administrator for the program.

29 CFR 1910.134(g)(1)(i)(A): Respirators with tight-fitting facepieces were worn by employees who had facial hair that came between the sealing surface of the facepiece and the face or that interfered with valve function:        a.)  Anchor an Oldcastle Company, Production areas- Employees were required to wear filtering facepiece respirators (3M Brand #9211+ Aura particulate respirator N95) during work tasks including, but not limited to, bagging various silica based products.  On or about July 27th, 2017, the seal between the tight-fitting respirator facepiece and an employees face was comprised by facial hair.

29 CFR 1910.134(g)(1)(iii): Employee(s) did not perform a user seal check each time they put on a tight-fitting respirator using the procedure in Appendix B-1 of 29 CFR 1910.134 or procedures recommended by the respirator manufacturer that the employer demonstrated were as effective as those in Appendix B-1:       a) Anchor an Oldcastle Company, Production areas- Employees were required to wear filtering facepiece respirators (3M Brand #9211+ Aura particulate respirator N95) during work tasks including, but not limited to, bagging various silica based products.  On or about July 27th, 2017, some employees were not performing user seal checks to ensure a proper seal between the respirator and the employees face.
