29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards: (a) On or about July 25, 2017, for employees exposed to fall hazards of up to approximately 35 feet on roof slopes up to 10 in 12 while removing and installing roofing materials without adequate fall protection systems in place at St. Anne's Catholic Church located at 1321 Braman Avenue, in Bismarck, North Dakota. Note: This company was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard which was contained in OSHA Inspection Number, 985952, Citation 2, Item 1 and was affirmed as final order on 01/07/2015, with respect to a workplace located at 222 Curtis Street in Rapid City, in South Dakota and OSHA Inspection Number, 910481, Citation 1, Item 2, and was affirmed as final order on 10/22/2013, with respect to a workplace located at 15231 Howe Street, in Omaha, Nebraska.
