29 CFR 1926.102(a)(1): 29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles.  On or about August 3, 2017, at Lot 30, 6265 Overlook Club Drive, Suwanee, Georgia:  Employees performing roofing work were exposed to struck-by hazards from wood chips and nails when they used nail guns without wearing eye protection.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about August 3, 2017, at Lot 30, 6265 Overlook Club Drive, Suwanee, Georgia:  a) Two employees performing roofing work from an approximately 10/12 slope roof were exposed to a fall hazard of approximately 11-feet to greater than 25-feet when they used ropes attached to a pelican hook that was attached to a carabiner that was directly connected to the webbing of rock climbing harnesses at the front of their waists for fall protection.   b) Two employees performing roofing work from an approximately 10/12 slope roof were exposed to a fall hazard of approximately 11-feet to greater than 25-feet when they did not tie off while working on the roof.

29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladders were not used at an angle such than the horizontal distance from the top support to the foot of the ladder way approximately one-quarter of the working length of the ladder (the distance along the ladder between the foot and the top support).  On or about August 3, 2017, at Lot 30, 6525 Overlook Club Drive, Suwanee, Georgia:  Four employees performing roofing work were exposed to a fall hazard of approximately 11-feet when they used an aluminum extension ladder that was set at an angle with its base approximately 8-feet from the building and top support.
