29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about October 3, 2017, at Lot 12, 6090 Overlook Club Circle, Suwanee. Georgia:  Two employees performing roofing work were exposed to a fall hazard of approximately 19-feet and greater when they used an aluminum extension ladder that was set two feet above the roof landing.

29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladder(s) were not used at an angle such that the horizontal distance from the top support to the foot of the ladder was approximately one-quarter of the working length of the ladder:  On or about October 4, 2017, at Lot 12, 6090 Overlook Club Circle, Suwanee, Georgia:  Two employees performing roofing work were exposed to a fall hazard of approximately 19-feet when they used an aluminum extension ladder was set with the base of the ladder approximately  7-feet 9-inches from the house.  Victor Carrera was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.1053(b)(5)(i), which was contained in OSHA inspection number 1252253, citation number 1, item number 3 and was affirmed as a final order on 10/27/2017, with respect to a workplace located at 6525 Overlook Club Drive, Suwanee, Georgia.
