29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation. a) On or about August 4, 2017: an employee operating a circular saw to trim wood was not wearing safety glasses, the employee was exposed to eye injuries. b) On or about August 4, 2017: an employee operating a nail gun to install bracing elements to roof trusses was not wearing safety glasses, the employee was exposed to eye injuries. c) On or about August 4, 2017: an employee operating a nail gun to install sheathing was not wearing safety glasses, the employee was exposed to eye injuries. d) On or about August 4, 2017: an employee operating a nail gun to install bracing elements to roof trusses was not wearing safety glasses, the employee was exposed to eye injuries.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) On or about August 3, 2017: two employees were marking and cutting wood trim along the edge of the roof to install the fascia were not protected by guardrail system, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection measure. Employees were exposed to a 18-feet fall hazard. b) On or about August 4, 2017: an employees was installing wood frame elements on roof trusses was not protected by guardrail system, safety net system, or personal fall arrest system, nor was employee provided with an alternative fall protection measure. Employee was exposed to a 12-feet fall hazard. P & S Service Group, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 697338, citation number 2, item number 1 and was affirmed as a final order on December 6, 2012, with respect to a workplace located at 511 Deercroft Ln., Jacksonville, FL 32222. P & S Service Group, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 761422, citation number 1, item number 2 and was affirmed as a final order on January 16, 2013, with respect to a workplace located at 9555 Baymeadows Road East Jacksonville, FL 32210.
