At a temporally jobsite, located at Ortega Lake Drive, lot 9 thru 12, in Jacksonville, Florida: On or about August 13, 2024, the employer exposed employees to fall injuries, in that the employer allowed employees to use the top step of a Werner 6' stepladder to gain access and egress the upper level of the roof.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a. Employees performing sheathing work at elevations of six feet or more above the ground are not being protected from fall hazards by a fall protection system: the employer P&S Service Group, Inc. fails to assure effective protection against fall as required by the standard, as demonstrated on August 13, 2024, when two (2) employees performing sheathing work on a 5:12 roof pitch of a new residential construction located at Ortega Lake Drive, lots 9 thru 12, in Jacksonville, FL, were exposed to the fall hazards, in that employees did not use fall protection system. P&S Service Group, Inc, was previously cited for a Repeat Serious violation of this occupational safety and health standard or its equivalent standard 29CFR 1926.501(b)(13), which was contained in OSHA inspection number 1642855, citation number 1, item number 1 and was affirmed as a final order on 07/19/23, with respect to a workplace located at 243 Albacete Road, in Saint Augustine, FL, 32084. P&S Service Group, Inc, was previously cited for a Repeat Serious violation of this occupational safety and health standard or its equivalent standard 29CFR 1926.501(b)(13), which was contained in OSHA inspection number 1496971, citation number 2, item number 1 and was affirmed as a final order on 03/21/22, with respect to a workplace located at 58 Lob Wedge Lane, in Bunnell, Fl, 32210.
