29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system.  1) 5 Chesterton Court, Barnegat, NJ: On or about 08/08/2017, two Giant Builders employees were conducting roofing activities, installing underlayment, on the steep sloped roof of a two story residential structure.  The employees were wearing harnesses, but were not attached to safety lines and were thus exposed to fall hazards between 10 and 20 feet from the roofs eves and rake edges to the ground below.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired;  1) 5 Chesterton Court, Barnegat, NJ: On or about 08/08/2017, two Giant Builders employees were conducting roofing activities on the steep sloped roof of a two story residential structure.  The employees used a portable aluminum extension ladder to access the roof.  The ladder was found to have a bent/twisted frame rail, exposing the employees to a potential fall hazard.
