In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).

29 CFR 1904.29(b)(4): 29 CFR 1904.29(b)(4): A form with equivalent information to OSHA's 301, Injury and Illness Incident Report, was not used by the employer to document each recordable injury and illness entered on the OSHA 300 Log. a) On or about 08/08/2017 - The employer failed to provided the OSHA 301 or equivalent for review. The employer provided the company accident report, which does not have the required information. The form failed to include information and the name and title of the person filling out the form. The form also fails to include the injured employees address, date of birth, time employee began working, their sex, or of the employee was treated in the emergency room. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).
