29 CFR 1910.147(f)(1): When lockout or tagout devices temporarily were removed from the energy isolating device and the machine or equipment was energized to test or position the machine, equipment or component thereof, the sequence of actions in 29 CFR 190.147(f)(1)(i) through 1910.147(f)(1)(v) were not followed: a) Job Site, South Wall: On or about May 16, 2022, and at times prior, the employer exposed an employee to struck-by hazards, in that the employer did not document or ensure the use of specific and effective control of hazardous energy procedures for the "Hany" Model IC 650 Grout Pump Plant (Serial Number GPL21009) for employees performing servicing and maintenance on this equipment.
