29 CFR 1904.39(a)(2): Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, the employer did not report the in-patient hospitalization, amputation, or loss of an eye to OSHA.  On Friday 4 August 2017 at approximately 8:30 AM, an employee setting up the HMC Bagger line suffered an amputation of the right index fingertip resulting in bone loss. The amputation was not reported within 24 hours to the local Area Office of the Occupational Safety and Health Administration. The Austin Area Office was notified on 7 August 2017 at approximately 3:00 PM.
