29 CFR 1910.303(b)(7)(iv): There were damaged parts that could adversely affect safe operation or mechanical strength of the equipment, such as parts that were broken, bent, cut, or deteriorated by corrosion, chemical action, or overheating    A.  Forge Floor, Forge Press #44:  On, or about, August 10, 2017, conduit leading to a junction box mounted on the front of the press above the foot pedal was broken, exposing the inner conductors.      B.  Steelyard, Saw #2:  On, or about, September 20, 2017, an electrical box mounted on the horizontal band saw in-feed conveyor had a broken latch and the door was ajar.        C.  Forge Floor, Trim Press #45:  On, or about, September 20, 2017, electrical conduit mounted on the front of the press above the operators station was broken, exposing the inner conductors.        D.  Forge Floor, Wall receptacle behind Forge #44:  On, or about, September 20, 2017, the lower, right receptacle was damaged and the outlet was sparking.

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating  at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures:    On, September 20, 2017, in the following locations, live electrical equipment was exposed to accidental contact:      A.  Steelyard, Saw #4:  Solenoid valves provided on the backside of the horizontal band saw were not provided with covers.  One of the valves had electrical tape wrapped around to cover the live contacts.  The others were left uncovered with live terminal screws and contacts.      B.  Switchgear Room for #44:  A cover was not provided on a Square D motor starter mounted next to panel LVP1.      C.  Switchgear Room for #48:  The cover provided on the controls for Press #48 was left open.

The J.J. Ryan Corporation Rex Forge Division was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA Inspection Number 910128, Citation Number 1, Item Number 7 and was affirmed as a Final Order on December 23, 2013, with respect to a workplace located at Plantsville, Connecticut.

29 CFR 1910.303(b)(7): Electric equipment was not installed in a neat and workmanlike manner:    On, or about, September 20, 2017, in the following locations, electrical equipment was not properly installed and/or maintained:      A.  Steelyard, Saw #3:  An electrical box mounted on the side of the horizontal band saw in-feed conveyor was provided with an improvised cover that appeared to be a piece of sheet metal.        B.  Steelyard, Saw #4:  The main electrical box provided on the side of the horizontal band saw was not secured shut.  Hinges had been added to the box and the door hung in the closed position from the hinges, it was not secured.        C.  Forge Floor, Press #43, Trim Press exit conveyor:  The electrical box provided for the conveyor did not match the installed control which left a gap around the control knob and access to the live electrical parts inside of the box.        D.  Forge Floor, Loader at Press #43:  An electrical box provided on the side of the loader, above the controls, was provided with a home-made cover that appeared to be fabricated out of sheet metal.      The J.J. Ryan Corporation Rex Forge Division was previously cited for a violation of this Occupational Safety and Health standard, which was contained in OSHA Inspection Number 910128, Citation Number 1, Item Number 8 and was affirmed as a Final Order on December 23, 2013, with respect to a workplace located at Plantsville, Connecticut.
