29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: Forge Production Area: On or about March 25, 2022, employees were using two forklifts simultaneously to conduct a below the tines lift in a free rigging configuration, because the 7 1/2-ton bridge crane used for this was out of service. A Caterpillar forklift, model number GP45N1, serial number AT29D60349 and Caterpillar forklift, model number GP30N, serial number AT13G02532, were used to move bar stock weighing between 6,000 lbs. and 8, 000 lbs., by attaching slings to the tines.

29 CFR 1910.184(c)(1): Sling(s) that were damaged or defective were used: Forge Production Area: On or about March 25, 2022, the web slings that were being used for below the tines lifts on forklifts, Tuff-Edge, type EE2-804T, serial number 1822347-01 and Tuff-Edge, type EE2-804T, serial number 1822347-05, were found to be damaged at the eyes, exposing employees to injuries from sling failure.
