29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: (a) 5701 MacCorkle Avenue SE, Charleston WV. On or about August 9, 2017, employees were exposed to falls of up to 13ft. 2in. to the concrete ground while performing roofing work on the edge of the flat drive thru roof with no fall protection. (b) 5701 MacCorkle Avenue SE, Charleston WV. On or about August 9, 2017, employees were exposed to falls up to 17ft. 4in. to concrete ground while performing roofing work on the flat main roof of the building with no fall protection. Peach State Roofing, Inc. was previously cited for a repeat, serious violation of this occupational safety and health standard or its equivalent standard, 1926.501(b)(10), Roofing Work on Low Slope Roofs, which was contained in OSHA inspection number 1066192, citation number 1, item number 1 and was affirmed as a final order on July 25, 2016, with respect to a workplace located at 2250 AL Highway 150, Hoover, Alabama.
