29 CFR 1926.552(a)(4): Hoisting ropes were not installed in accordance with the wire rope manufacturers'  recommendations:  On or about 8/9/2017 at 5035 Mirror Lake Drive, Lot 205, Cumming, GA; where the employer failed to ensure the material hoist ad the proper type rope installed where the prescribed wire type rope was replaced with a nylon "climbing" rope used to lift materials to the roof area exposing workers below to potential failure if the rope were to break or the knot loosen.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   On or about 8/9/2017 at 5035 Mirror Lake Drive Lot 205 Cumming, GA; where employees were exposed to fall hazards of 21-feet to the ground while performing roofing operations at the edge of the eaves and were not connected.  H&J Roofing, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 989594, citation number 01, item number 01 and was affirmed as a final order on 10/27/2017, with respect to a workplace located at 1511Silver Mist Circle, Lot 176, Powder Springs, GA 30127.
