29 CFR  1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles:  New residential construction located on Lot #18 Peppertree Drive, Newnan, GA 30265, Roof:  On or about 11/04/2020, employees tasked with installing shingles to a 4:12 pitched roof were not protected from struck by hazards, in that protective eyewear was not provided nor used while operating pneumatic nail guns.

29 CFR  1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   On or about 11/04/2020, during the installation of roofing material on a newly constructed residential property located at Lot #18 Peppertree Drive, in Newnan, GA, the employer failed to ensure that employees were protected from fall hazards by conventional fall protection and/or devices. Employees were exposed to fall hazards from heights of approximately approximately10 feet, 6 inches, from the top of the roof to ground level.

29 CFR  1926.502(d)(17): The attachment point of the body belt was not located in the center of the wearer's back. The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head.   On or about 11/04/2020, during the installation of roofing material on a newly constructed residential property located at Lot #18 Peppertree Drive in Newnan, GA, employees were exposed to fall hazards of  approximately 10 feet, 6 inches in that, a personal fall arrest system with the life-line connected near the wearer's back was not utilized.  The exposed employee used a mountain climbing rope attached to the waist area as a positioning device.  A full body harness with a D-Ring was not used as an approved fall protection system for roofing activities.  H & J Roofing LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (name previously cited standard), which was contained in OSHA inspection number 1256120, citation number 1, item number 2 and was affirmed as a final order on 01/30/3018, with respect to a workplace located at 5035 Mirror Lake Drive (Lot #205), Cumming, GA .
