The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head.   On or about 11/04/2020, during the installation of roofing material on a newly constructed residential property located at Lot #18 Peppertree Drive in Newnan, GA, employees were exposed to fall hazards of  approximately 10 feet, 6 inches in that, a personal fall arrest system with the life-line connected near the wearer's back was not utilized.  The exposed employee used a mountain climbing rope attached to the waist area as a positioning device.  A full body harness with a D-Ring was not used as an approved fall protection system for roofing activities.  H & J Roofing LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (name previously cited standard), which was contained in OSHA inspection number 1256120, citation number 1, item number 2 and was affirmed as a final order on 01/30/3018, with respect to a workplace located at 5035 Mirror Lake Drive (Lot #205), Cumming, GA .
