29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a) On or about 08/17/17, on the roof of a house at 71 Mill Street, Williamsville, NY: An employee engaged in roofing work on a steep roof was exposed to a fall of approx. 18 ft. No fall protection system was being used. Pitch of the roof was approx. 5 in 12. NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: a) On or about 08/17/17, on the roof replacement jobsite on a house at 71 Mill Street, Williamsville, NY: An employee working on the ground doing clean-up work in the vicinity of employees working above on the roof was exposed to the potential hazard of being struck by materials/tools that could fall off the roof. The employee was not wearing a hardhat. NO ABATEMENT CERTIFICATION REQUIRED
