29 CFR 1926.20(b)(2): Such programs did not provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers. On 7/25/2017, two sub-contractors of Cain Hernandez were removing and replacing roofing materials from a roof of a commercial building located at 119 East Main Street in Warrenton, Missouri. Cain Hernandez did not conduct an adequate inspection of the job site, materials, and equipment. During the inspection, employees were exposed to the following hazards: a) A subcontractor employee was not wearing eye protection while using a pneumatic nail gun to install sheathing. b) Several subcontractor employees were not wearing protective helmets while working beneath other employees installing roofing materials directly above them. The employees were subject to head injuries from possible falling tools and supplies. c) The subcontractor owners as well as their employees were observed on the roof of the building approximately ten feet above the next lower level without utilizing any fall protection.

On 7/25/2017, employees and subcontractors of Cain Hernandez were removing and replacing roofing materials shingles from a roof of a commercial building located at 119 East Main Street in Warrenton, Missouri. The foreman and two employees were not wearing protective helmets while working beneath the subcontractor employees as they were removing and installing materials from the roofs directly above them.
