Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest systems. On 8/14/2017, employees from Auchly Roofing, Inc. were installing shingles on a roof of a residential building located at 113 Christina Marie Drive in OFallon, MO. They were not utilizing guardrail systems, a safety net system, or personal fall arrest systems. The owner and three employees were observed on a 6/12 sloped roof of a residence approximately nine feet above the ground without fall protection, exposing them to a fall to the ground below.
