Employees were not able to recognize fall hazards and the procedures necessary to reduce or eliminate these hazards.

On 4/5/2019, employees of Auchly Roofing, Inc. were working on a residential building located at Lot 151 Timber Bluff Drive, Wentzville, MO. Employees were installing shingles approximately 13 to 17 feet above a lower level. Employees were not utilizing guardrail systems, a safety net system, or a personal fall arrest system. The employees were observed on the roof of the building without fall protection. Auchly Roofing, Inc. was previously citied for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1256473, citation number 1, item number 2 and was affirmed as a final order on 7/10/2018, with respect to a workplace located at 113 Christina Marie Drive, Ofallon, MO.
