29 CFR 1926.405(b)(2): All pull boxes, junction boxes, or fittings were not provided with covers and in energized installations each outlet box did not have a cover, faceplate, or fixture canopy: a) Superintendent Room: The light switch adjacent to the doorway on the wall was energized and did not have a cover plate affixed. Employees were exposed to electrical shock hazards. b) Superintendent Room: The outlet adjacent to the doorway was energized and did not have a cover plate affixed. Employees were exposed to electrical shock hazards. c) Bathroom: The outlet located on the tiles was energized and did not have a cover plate affixed. Employees were exposed to electrical shock hazards. d) Criminal Investigation Room: The light switch adjacent to the doorway on the wall was energized and did not have a cover plate affixed. Employees were exposed to electrical shock hazards. e) Hallway: The outlet powering the extension cord for the floor fan was energized and did not have a cover plate affixed. Employees were exposed to electrical shock hazards. f) Hallway: The outlet located behind the Werner stepladder was energized and did not have a cover plate affixed. Employees were exposed to electrical shock hazards. g) Storage Room: The outlet located adjacent to the sink was energized and did not have a cover plate affixed. Employees were exposed to electrical shock hazards.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: Hallway: The extension cord powering the Lasko floor fan had the external jacket pulled back from the plug end. Strain relief was not maintained. Employees were exposed to electrical shock hazards.
