29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval:  a)  At the facility, on or about and at times prior to August 30, 2017, the employer drilled a hole in the overhead protection pole attached to the Toyota SAS #1091,  #1553 and Crown #1618 forklifts without pre-approval from the forklift manufacturers. The employer drilled the holes in the pole to install a portable fire extinguisher, thus exposing employees to struck-by hazards.
