29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: Three employees performing electric cable installation work were exposed to cave-in hazards while working inside an unprotected trench that did not have a ladder installed inside the trench box to provide safe access and egress.

29 CFR 1926.651(j)(2): Protection was not provided by placing and keeping excavated or other materials or equipment at least 2 feet (.61m) from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary. On or about August 29, 2017, at 3260 Lawrenceville-Suwanee Road, Suwanee, Georgia: Three employees performing electric cable installation work were exposed to struck-by hazards from the spoil pile, tools, and materials that were placed within a foot of the excavation. The spoil pile and materials were not set at least two feet of the excavation.

On or about August 29, 2017, at 3260 Lawrenceville-Suwanee Road, Suwanee, Georgia: Three employees performing electric cable installation work were exposed to cave-in hazards while working inside an unprotected trench that was approximately 21-feet long, 4.5-feet wide, and 5-feet 7-inches deep on the street side and 6-feet deep on the opposite side.
