29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) Machine Shop: On or about August 31, 2017 and times prior, the employer did not ensure that machine guarding was provided on The Monarch Machine Tool Company lathe (MFGR'S NO. 30883) to protect employees from the rotating chuck. (b) Machine Shop: On or about August 30, 2017 and times prior, the employer did not ensure that machine guarding was provided on a Chas. G. Allen Company drill press (ID 0108146) to protect employees from the rotating chuck. (c) Machine Shop: On or about July 31, 2017 and times prior, the employer did not ensure that machine guarding was provided on an Edlund Machine Company drill press (Model 1F 7; Serial No. 603) to protect employees from the rotating chuck. (d) Machine Shop: On or about August 31, 2017 and times prior, the employer did not ensure that adequate machine guarding was provided on a WF Wells and Sons, Inc. horizontal bandsaw (Model L 62574) to protect employees from the unused portion of the blade.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch: (a) Machine Shop: On or about August 30, 2017 and times prior, employees operated Black & Decker Mfg. Co. 10" Ball Bearing Bench Grinder (Serial No. D 2822471) that had an approximately 7/8 inch openings between the tongue guard and the abrasive wheel periphery on both sides.
