29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):    a)  On August 31, 2017, employees were exposed to the hazard of falling approximately 27 feet to the ground, while performing masonry work from tubular welded frame scaffold platforms that were not fully planked.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.100(a):  Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:    a) On August 31, 2017, employees were not wearing protective helmets where there was a possible danger of head injury.  PMM Global LLC, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.100(a), which was contained in OSHA inspection number 1109913, citation number 1, item number 1 and was affirmed as a final order on February 22, 2016, with respect to a workplace located at 3821 N. Southport Ave., Chicago, Illinois 60613.  No abatement certification or documentation is required for this item.
