29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) On or about 8/28/2017, inside the facility, in the single phase production area of line 2, employees standing on a step stand and removing jammed paper from within the rotary sheeter machine while it was running were exposed to an ingoing nip point hazard.

29 CFR 1910.219(e)(1)(i): Horizontal belts which had both runs 42 inches or less from the floor level were not fully enclosed by guards conforming to requirements specified in 29 CFR 1910.219(m) and (o): a) On or about 8/28/2017, inside the facility, in the single phase production area of line 2, employees accessing a step stand adjacent to the side of the rotary sheeter were exposed to a caught-in hazard from an approximately 9 by 7 inch hole in an expanded metal guard covering a belt and pulley.
