29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c): (a) Avalon Crossing II located at 2855 Old Milton Pkwy., Alpharetta, GA 30009, Excavation: On or about 09/05/2017, and at times prior, the employer exposed employees to cave-in hazards while working in a 11-foot, 11-inch deep trench, in that a cave-in protection system was not provided in the location where an employee was installing a 6-inch PVC pipe.
