29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury: a) Job site, 230 Pearce Mill Road, Wexford, PA: On or about September 6, 2017, during paving operations, the employer did not ensure that appropriate training/instructions were provided in that employees were not kept from mobile paving operations danger areas where hazards such as rotating augers and screed crushing hazards were present.
