29 CFR 1926.501(b)(1):  Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:        a)  On September 7, 2017, employees performing masonry work were exposed to fall hazards of approximately 11.8 feet from the edge of the steel frame of the building.

29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds were not fully planked or decked between the front uprights and the guardrail supports:        a) On September 7, 2017, employees were exposed to the hazard of falling up to approximately 14.2 feet from tubular welded frame scaffold platforms which were not fully planked.     ADV Masonry Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(b)(1), which was contained in OSHA inspection number 1242060, citation number 1, item number 2 and was affirmed as a final order on August 16, 2017, with respect to a workplace located at 599 Ogden Ave., Lisle, IL 60532.
