29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: (a) An employee working in a seven foot deep trench used a ladder to access the upper landing surface of the trench which did not extend at least three feet above the upper landing area.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c). (a) The employer did not ensure that an adequate protective system was used in a trench to protect employees from potential trench collapse hazards. American Sewer Services, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.652(a)(1), which was contained in OSHA inspection number 1177757, citation number 2, item number 1 and was affirmed as a final order on February 27, 2017, with respect to a workplace located at 200 W. St. Paul Ave, Milwaukee WI 53203. American Sewer Services, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.652(a)(1), which was contained in OSHA inspection number 1153906, citation number 1, item number 1 and was affirmed as a final order on August 9, 2016, with respect to a workplace located at Hwy 164 & Blackhawk Trail, Waukesha WI 53186.
