29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle: a. On or before September 11, 2017, the point of operation including the clamp and pressure dies on the Comco CNC pipe bender (model: KB-70NS; S/N: K-02707-N) was not guarded, thereby exposing employees to caught-in and struck-by hazards. On or about September 5, 2017, the tips of an employee's right ring and middle fingers were amputated due to getting caught in the clamp die while operating the Comco CNC pipe bender.

29 CFR 1904.29(a): Basic requirement. You must use OSHA 300, 300-A, and 301 forms, or equivalent forms, for recordable injuries and illnesses. The OSHA 300 form is called the Log of Work-Related Injuries and Illnesses, the 300-A is the Summary of Work-Related Injuries and Illnesses, and the OSHA 301 form is called the Injury and Illness Incident Report: a. On or before September 11, 2017, the employer did not use the OSHA 300 Log of Work-Related Injuries and Illnesses or an equivalent form for recordable injuries and illnesses in the workplace. The employer's 2017 OSHA/Worker's Compensation Claims log did not contain the employer's city and state, where the event occurred, columns for classifying the case equivalent to OSHA's columns G through J, columns for the injury or type of illness equivalent to OSHA's columns M(1) through M(6), separate logs for each year, and the page totals.
