On or about December 09, 2025, located at 2025 Camaro Avenue, Columbus Ohio, 43207:  The employer failed to assess the phosphating area for hazards which necessitate the use of personal protective equipment.  The task of spraying Tectyl 859B was not evaluated to identify personal protective equipment needed to protect the eyes and face of employees.  The employers hazard assessment identified safety glasses as the only means required for protecting employees eyes, exposing employees to eye and skin irritation if splashed by Tectyl 859B.

On or about December 09, 2025 and times prior, located at 2025 Camaro Avenue, Columbus Ohio, 43207: Maintenance employees were exposed to, but not limited to, electrical shock hazards, caught in hazards, and crush by hazards when working on equipment in the facility. The employer did not develop clear and specific procedures for the control of hazardous energy prior to conducting the work on equipment including but not limited to the Large Radius Bender, KB70, KBB70, KMT360SA and SP-90. The procedures did not contain adequate information to include the following:  (1)  A specific statement of the intended use of the procedure;  1910.147(c)(4)(ii)(A).  (2)	Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy; (c)(4)(ii)(B)].  (3)	Specific procedural steps for the placement, removal of lockout devices or tagout devices, specifically "Do not operate" tags on the equipment [(c)(4)(ii)(C)].

29 CFR  1910.333(b)(2)(iv)(B): A qualified person shall use test equipment to test the circuit elements and electrical parts of equipment to which employees will be exposed and shall verify that the circuit elements and equipment parts are deenergized. The test shall also determine if any energized condition exists as a result of inadvertently induced voltage or unrelated voltage backfeed even though specific parts of the circuit have been deenergized and presumed to be safe. If the circuit to be tested is over 600 volts, nominal, the test equipment shall be checked for proper operation immediately after this test.   a. On or around November 24, 2025, located at 2025 Camaro Avenue, Columbus Ohio, 43207: The employer failed to ensure that electrical safety-related work practices were implemented to to prevent electrical shock or other injuries resulting from direct or indirect contact when a employee worked near an energized electrical circuit. The SP90 and KB70 benders were not deenergized prior to performing blowouts inside an open 208-volt electrical cabinet, exposing the employee to burn hazards from an arc flash and blast as a result.
