29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.  a. On or about November 30, 2023, and at times prior, employees operating the Chiyoda SP-90 bender (model: SP-90S; S/N: 8507) were not protected from the hazards associated with the reciprocating motion of the carriage and the pinch points between the carriage and the rails as well as the pinch points between the collet pads during the complete operation cycle, thereby exposing employees to caught-in and struck-by hazards.  b. On or about November 29, 2023, and at times prior, employees operating the Comco KB-70 bender (model: KB-70NS; S/N: K-02707-N)  were not protected from the hazards associated with the reciprocating motion of the carriage and the pinch points between the carriage and the rails as well as the pinch points between the collet pads during the complete operation cycle, thereby exposing employees to caught-in and struck-by hazards.

29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.   a. On or about November 30, 2023, and at times prior, the employer did not ensure that the points of operation including, but not limited to, the pressure die, clamp die, and bend die on the Chiyoda SP-90 bender (model: SP-90S; serial no: 8507) were guarded during the complete operation cycle, thereby exposing employees to caught-in and struck-by hazards.   b. On or about November 30, 2023, and at times prior, the employer did not ensure that the points of operation including, but not limited to, the pressure die, clamp die, and bend die on the Comco KB-70 bender (model: KB-70NS; serial no: K-02707-N) were guarded during the complete operation cycle, thereby exposing employees to caught-in and struck-by hazards.   Appian Manufacturing Corporation was previously cited for a Repeat-Serious violation of this Occupational Safety and Health standard 29 CFR 1910.212(a)(3)(ii), which was contained in OSHA inspection number 1426484, citation number 1, item number 1 and was affirmed as a final order on April 30, 2020, with respect to a workplace located at 2025 Camero Avenue, Columbus, Ohio 43207.

29 CFR 1910.147(c)(6)(i): The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed.  a. On or about November 30, 2023, and at times prior, the employer did not ensure that periodic inspections were performed at least annually with authorized employees who performed maintenance and servicing covered tasks on machinery throughout the facility, including, but not limited to, the Chiyoda SP-90 bender (model: SP-90S; serial no: 8507).  b.
