29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees. Production area #2 Mill Line - On or about December 12, 2017, locks or other hardware for the purpose of isolating the energy sources were not used by the employees responsible for performing services such as, but not limited to set-up/changeover. Employees are thereby exposed to hazards associated with moving parts and/or the unintentional start-up of the equipment.
