29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b): a) Jobsite, 331 Lincoln Drive, Pittsburgh, PA 15241: Observed on or about September 7, 2017, employees engaged in roofing work were not protected from falling by a guardrail system, safety net system, or a personal fall arrest system. The slope of the roof was approximately 4 in 12 (vertical to horizontal). The employees were exposed to a fall hazard of approximately 15 feet to the ground below.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: a) Jobsite, 331 Lincoln Drive, Pittsburgh, PA 15241: Observed on or about September 7, 2017, the ladder side rails on a portable ladder used to access the roof of the house did not extend at least 3 feet above the upper landing surface. Employees using the ladder to access the roof were exposed to a fall hazard of approximately 15 feet to the ground below.
