29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    (a)  Jobsite:  On or about March 24, 2018, two employee were using an aluminum extension ladder that extended less than two feet above the edge of the roof.  The employee were using this ladder to access/egress the roof.  The one employee was also using this ladder to carry shingles (one bundle at a time) to the roof.   The Evans Roofing and Gutters Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 1271495, citation number 1, item number 3 and was affirmed as a final order on December 5, 2017, with respect to a workplace located at 103 Eaglebrook Court, Venetia, PA 15367.   The Evans Roofing and Gutters Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 1264151, citation number 1, item number 2 and was affirmed as a final order on November 14, 2017, with respect to a workplace located at 331 Lincoln Drive, Pittsburgh, PA 15241.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a)  Jobsite:  On or about March 24, 2018, two employees were working from a residential roof and they were not protected from falling to the ground.  The fall hazard to the two employees was up to approximately 16 feet.  The employees were replacing an asphalt shingle roof.

29 CFR 1926.503(c): When the employer has reason to believe that any affected employee who has already been trained does not have the understanding and skill required by paragraph (a) of this section, the employer is to retrain each such employee such as inadequacies in an affected employee's knowledge or use of fall protection systems or equipment indicate that the employee has not retained the requisite understanding of skill:   (a) Jobsite:  On or about March 24, 2018, the two employees that were working on the residential roof and were exposed to a fall hazard of up to 16 feet need to be retrained on when, where and how to use fall protection when working from a roof.
