29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe condition(s) and the regulation(s) applicable to his work environment to control or eliminate any hazard(s) or other exposure to illness or injury:  a) Jobsite, 103 Eaglebrook Court, Venetia, PA:  On or about October 18, 2017, 3 employees were engaged in roofing activities, which included, but not limited to, climbing ladders, using power tools, working at heights, and the employer did not produce a safety and health program that would have included hazard recognition and abatement of those hazards.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer:  a) Jobsite, 103 Eaglebrook Court, Venetia, PA:  On or about October 18, 2017, 3 employees were working at heights of 30-35 feet on a residential roof, without fall protection.  The owner did not respond to the subpoena request for the company's fall protection training records for the employees.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) Jobsite, 103 Eaglebrook Court, Venetia, PA:  On or about October 18, 2017, three employees were using a extension ladder to access the roof that was not extended three feet above the landing surface.  The fall hazard to the employees was approximately 20 feet to the ground below.
