29 CFR 1904.4(a): Basic requirement. Each employer required by this Part to keep records of fatalities, injuries, and illnesses must record each fatality, injury and illness that is work related. a) On September 18, 2017 a recordable injury occurred when an employee's foot was caught in between the attachment bar and the cab frame of a Kubota manure spreader. The employer did not record the injury on the required 300 logs.

29 CFR 1904.39(a)(2): Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA. a) On September 18, 2017, an employee was hospitalized for a work related injury and the employer did not report a reportable injury to OSHA within the required twenty-four hours.
