29 CFR 1928.57(a)(6): Operating instructions. At the time of initial assignment and at least annually thereafter, the employer shall instruct every employee in the safe operation and servicing of all covered equipment with which he is or will be involved, including at least the following safe operating practices: 29 CFR 1928.57(a)(6)(i): Keep all guards in place when the machine is in operation; a. On or before April 22, 2019, where an employee was injured by a rotating Power Take Off shaft between a tractor and sand wagon, the employee was not instructed about the guarding requirements of the Power Take Off shaft, resulting in an injury that required the overnight hospitalization of the employee.

29 CFR 1928.57(b)(1)(iii): Power take-off driven equipment shall be guarded to protect against employee contact with positively driven rotating members of the power drive system. Where power take-off driven equipment is of a design requiring removal of the tractor master shield, the equipment shall also include protection from that portion of the tractor power take-off shaft which protrudes from the tractor. a. On or before April 30, 2019, the sand wagon hydraulic pump and Power Take-Off shaft coupling was unguarded, which resulted in an employee injury on April 22, 2019 requiring the overnight hospitalization of the employee due to the injuries received from being caught in the rotating Power Take-Off shaft.
