On or about January 18, 2025, employees were exposed to fall hazards of approximately 12 feet to the ground while working on top of the silage pile engaged in plastic and tire removal process.

The OSHA 300 form is called the Log of Work-Related Injuries and Illnesses, the 300-A is the Summary of Work-Related Injuries and Illnesses, and the OSHA 301 form is called the Injury and Illness Incident Report. a. On or about January 18, 2025, an employee suffered serious injuries when they fell approximately 12 feet from the top of a silage pile resulting in days away from work. The employer did not ensure the injury was recorded on the OSHA Injury and Illness forms.

Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA. a. On or about January 18, 2025, an employee was hospitalized with serious injuries when they fell approximately 12 feet from the top of a silage pile. The employer did not report the in-patient hospitalization to OSHA within 24 hours.
