29 CFR 1910.28(b)(1)(i): Except as provided elsewhere in this section, the employer must ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the following:  29 CFR 1910.28(b)(1)(i)(A): (A) Guardrail systems.  a. On or before October 4, 2017 where employees used the loading ramp on the east side of the building, the guardrail system was broken from its supports, thereby exposing employees to a fall hazard.

29 CFR 1910.176(a): Use of mechanical equipment. Where mechanical handling equipment is used, sufficient safe clearances shall be allowed for aisles, at loading docks, through doorways and wherever turns or passage must be made. Aisles and passageways shall be kept clear and in good repair, with no obstruction across or in aisles that could create a hazard. Permanent aisles and passageways shall be appropriately marked.  a. On or before October 4, 2017 where employees operated powered industrial vehicles in the warehouse, the employer did not maintain the aisles in good repair, such as the broken concrete floor in the aisle located between row 100 and 200.  b. On or before October 4, 2017 where employees operated powered industrial vehicles in the warehouse, the employer did not maintain the aisles in good repair, such as the broken concrete floor in the aisle located between row 300 and 400.
