29 CFR 1910.107(b)(9): A clear space of not less than 3 feet on all sides was not kept free from storage or combustible construction: SPRAY COATING On, or about, October 4, 2017, the flammable spray booth had material stored within the three foot clear space around the booth, which included, a flammable storage cabinet, storage lockers that contained paper and other combustible material when employees used flammable materials, such as, but not limited to PPG AQUACRON Primer, which had a flash point of 122F.

29 CFR 1910.134(c)(2)(ii): The employer did not establish and implement those elements of a written program necessary to ensure that any employee using a respirator voluntarily was medically able to use that respirator, and that the respirator was cleaned, stored, and maintained so that its use does not present a health hazard to the user: GRINDING On, or about, December 13, 2017, an employee was not medically evaluated to determine the ability to wear a 3M, P100, half face, elastomeric respirator for voluntary use while performing grinding activities.
