29 CFR 1910.178(a)(4):  Capacity, operation, and maintenance instruction plates, tags or decals of the powered industrial truck were not changed accordingly when modifications and additions which affect capacity and safe operation of powered industrial trucks:  Production Floor:  On, or about, March 7, 2019, tip hoppers were used throughout the facility to collect scrap.  The tip hoppers were then transported using a forktruck to the waste collection area.  The tip hopper observed in the Screw Department contained mild steel scrap, a relatively heavy material.  The employer had not determined the safe load limit when tip hoppers were used with forktrucks in the facility.

Die Cell:  On, or about, March 7, 2019, the rotating chip conveyor provided on the Kuraki KBT 13 milling center was not provided with safeguarding.  It was located in the walking surface of the work platform and was operated with a standard on/off switch.     C.  Die Cell:  On, or about, March 7, 2019, the Mazak Integrex 70 turning center was not provided with safeguarding such as an interlocked access door. D.  Die Cell:  On, or about, March 7, 2019, the Mazak Integrex 70YB turning center was not provided with safeguarding such as an interlocked access door.   E.  Die Cell:  On, or about, March 7, 2019, safeguarding was not provided on the Okuma horizontal lathe lead screw. F. 29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):   A.  Fabrication Department:  On, or about, February 25, 2019, an employee suffered multiple partial finger amputations while attempting to cut copper plate on the Large Allsteel Shear.  The barrier guard provided on the shear was not installed in accordance with guard opening guidelines in that the opening left between the guard and bed of the shear allowed access to both the hold-downs and shear blade.  B.  Fabrication Department:  On, or about, March 7, 2019, the barrier guard provided on the Small Allsteel Shear was not installed in accordance with guard opening guidelines to prevent access to the hold-downs.    C.  Fabrication Department:  On, or about, March 7, 2019, guards were not provided on the angle cutting station of the Austin Hastings iron worker, Model 70W-1019.  Also, the plexiglass guard provided at the hole punch allowed access to the point of operation.
