On or about October 6, 2017, during the installation of roofing material at the job site located at 3557 S. Arlington Rd, Akron, Ohio 44312; the employer did not ensure employees working on the roof of a commercial location (PNC Bank) were protected from possible danger of eye injury from impact, or from flying objects from the employees installing roofing material.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: On or about October 6, 2017, during the installation of roofing material at a job site located at 3557 S. Arlington Rd, Akron, Ohio 44312; the employer did not ensure employees working on a roof of a commercial location (Bank) were protected from a fall of approximately 12.1 feet by utilizing a fall protection system. The Boak & Sons Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.501(b)(11) Duty to have fall protection), which was contained in OSHA inspection number 961573, citation number 01, item number 01 and was affirmed as a final order on 6/11/2014, with respect to a workplace located at 5575 Cernic Pl, Austintown, Ohio, 44515.
