29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket). Visitor's Center, back-up generator room: On or about 11 October 2017 and times prior thereto, strain relief was not provided for a Avenger Steam Cleaner, Model AV12QX, serial number not legible. The outer protective jacket for the power supply cord was damaged in two locations. This condition exposed employees to electrical shock hazards and fire hazards.

29 CFR 1910.305(g)(1)(iv)(B): Flexible cords and cables were used where run through holes in walls, ceilings, or floors. Visitor's Center, Reception Area: On or about 11 October 2017 and times prior thereto, a 25-foot energized beige colored extension cord was routed through a window opening and further routed through a doorway and plugged into a desktop lamp. This condition exposed employees to an electrical shock and electrical burn hazard and fire hazards. The U.S. Forest Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.305(g)(1)(iv)(B) which was contained in OSHA inspection number 987418, citation number 1, item number 3 and was affirmed as a final order on 03 September 2014, with respect to a workplace located at Kenai Lake Work Center in Seward, Alaska.
