29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a) HEM Horizontal bandsaw fabrication/welding area Near bay door: On or about October 10, 2017 and times prior, employees used a HEM horizontal bandsaw to cut pipes and materials down to size.  The blade was not guarded to protect employees from the unused portion of the blade.    b) Marvel Vertical bandsaw fabrication/welding area across from Machine shop: On or about October 10, 2017 and times prior, employees used a Marvel vertical bandsaw to cut pipes and materials down to size.  The blade was not guarded to protect employees from the unused portion of the blade.    c) HEM Horizontal bandsaw machine shop area: On or about October 10, 2017 and times prior, employees used a HEM horizontal bandsaw to cut pipes and materials down to size.  The blade was not guarded to protect employees employees from the unused portion of the blade.    d) Bridgeport Drill Press machine shop area: On or about October 10, 2017 and times prior, employees used a Bridgeport Drill press to tap flanges and drill plates for supports.  The rotating drill bit was not provided with a guard to prevent employee exposure to the flying chips and rotating parts.     e) Niger Drill Press machine shop area: On or about October 10, 2017 and times prior, employees used a Bridgeport Drill press to tap flanges and drill plates for supports.  The rotating drill bit was not provided with a guard to prevent employee exposure to the flying chips and rotating parts.     f) Doya Drill Press machine shop area: On or about October 10, 2017 and times prior, employees used a Bridgeport Drill press to tap flanges and drill plates for supports.  The rotating drill bit was not provided with a guard to prevent employee exposure to the flying chips and rotating parts.     g)Gamet 12" lathe front left corner of machine shop: On or about September 26, 2017 and times prior, employees used a Gamet 12" lathe to place bevels in parts.  The rotating chuck was not provided with a guard to prevent employee exposure to the rotating parts.     h) 32" Lathe Back wall of machine shop far left side: On or about October 10, 2017 and times prior, employees used a 32" lathe (unknown make/model) to drill pipes and parts.  The rotating chuck was not provided with a guard to prevent employee exposure to the rotating parts.     i) 24" Lathe Back wall of machine shop center lather: On or about October 10, 2017 and times prior, employees used a 24" lathe (unknown make/model) to drill pipes and parts.  The rotating chuck was not provided with a guard to prevent employee exposure to the rotating parts.    j)12" Lathe Back wall of machine shop far right side: On or about October 10, 2017 and times prior, employees used a 12" lathe (unknown make/model) to drill pipes and parts. 29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch:    a) Stainless area: On or about September 10, 2017 and times prior, employees used the Allen Bradley bench grinder to sharpen drill bits.  The adjustable work rest was not adequately adjusted on the right side resulting in a 1 1/2 inch opening between the abrasive wheel and the work rest.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:  a) Stainless area: On or about September 10, 2017 and times prior, employees used a Allen Bradley abrasive bench grinder to sharpen drill bits.  The grinder did not have an adjustable tongue guard installed on the right side resulting in a 1 1/4 inch opening between the abrasive wheel and the housing.  b) Fabrication/Welding area: On or about October 3, 2017 and times prior, employees used a Allen Bradley abrasive bench grinder to sharpen tools and drill bits.  The grinder did not have the adjustable tongue guard on the left side adequately adjusted resulting in a 3/4 inch opening between the abrasive wheel and the tongue guard.
