Dow Bandsaw (SN - 254587): On or about March 2, 2023 and times prior, employees changed the bandsaw blade on the Bay 2 DOW Line Saw. The employer has not developed an energy control procedure for the Bay 2 DOW Line Saw exposing employees to amputation hazards from the unexpected start up during maintenance and servicing activities. (b) Machine Shop - HEM INC. Bandsaw (SN - 41977): On or about February 17, 2023 and times prior, employees changed the bandsaw blade on the Bay 2 Main Saw. The employer has not developed an energy control procedure for the Bay 2 Main Saw exposing employees to amputation hazards from the unexpected start up during maintenance and servicing activities.

The employee powered down the machine and changed the blade without applying a lockout device exposing employees to amputation hazards from the unexpected start up during maintenance and servicing activities.
