29 CFR  1910.147(c)(4)(i):  Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  (a) Machine Shop - HEM INC. Dow Bandsaw (SN - 254587):  On or about March 2, 2023 and times prior, employees changed the bandsaw blade on the Bay 2 DOW Line Saw.  The employer has not developed an energy control procedure for the Bay 2 DOW Line Saw exposing employees to amputation hazards from the unexpected start up during maintenance and servicing activities.  (b) Machine Shop - HEM INC. Bandsaw (SN - 41977): On or about February 17, 2023 and times prior, employees changed the bandsaw blade on the Bay 2 Main Saw.  The employer has not developed an energy control procedure for the Bay 2 Main Saw exposing employees to amputation hazards from the unexpected start up during maintenance and servicing activities.

Dow Bandsaw (SN - 254587):  On or about March 2, 2023 and times prior, employees changed the bandsaw blade on the Bay 2 DOW Line Saw.  The employee powered down the machine and changed the blade without applying a lockout device exposing employees to amputation hazards from the unexpected start up during maintenance and servicing activities.  (b) Machine Shop - HEM INC. Bandsaw (SN - 41977): On or about February 17, 2023 and times prior, employees changed the bandsaw blade on the Bay 2 Main Saw.  The employee powered down the machine and changed the blade without applying a lockout device exposing employees to amputation hazards from the unexpected start up during maintenance and servicing activities.

(a) Machine Shop - HEM INC. The employer has not ensured employees have been provided training on energy control procedures for changing the Bay 2 DOW Line Saw blade, exposing employees to amputation hazards from the unexpected start up during maintenance and servicing activities. 29 CFR  1910.212(a)(1):   One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:   (a) Machine Shop - HEM INC. Dow Bandsaw (SN - 254587): On or about March 3, 2023, an employee used the "Bay 2 Dow Line Saw" without a properly adjusted guard, exposing employee to rotating blade which could result in amputation.  (b) Machine Shop - HEM INC. Bandsaw (SN - 41977): On or about March 3, 2023, an employee used the "Bay 2 Main Saw" without a properly adjusted guard, exposing employee to rotating blade which could result in amputation.  (c) Machine Shop - HEM INC. Bandsaw (SN - 86979): On or about March 3, 2023, an employee used the "Secondary Saw" without a properly adjusted guard, exposing employee to rotating blade which could result in amputation.  (d) Machine Shop - Conrac Corporation Flaring Machine (SN - 84309-11): On or about March 3, 2023, an employee used the "Flaring Machine" without a properly adjusted guard, exposing employee to a rotating pin which could result in amputation.

29 CFR  1910.215 (a)(4):  Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch:  a) Machine Shop - Hisey Wolf Machine Pedestal Grinder: On or about March 2, 2023, and times prior, employees used the Hisey Wolf Machine abrasive pedestal grinder to sharpen tungsten rods.  The pedestal grinder had a maximum opening of 1 and a 1/2 inches between the grinding wheel and the work rest on the left side.

29 CFR  1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:  a) Machine Shop - Hisey Wolf Machine Pedestal Grinder: On or about March 2, 2023, and times prior, employees used the Hisey Wolf Machine abrasive pedestal grinder to sharpen tungsten rods.  The pedestal grinder had a maximum opening of 1 and 1/2 inches between the grinding wheel and adjustable tongue guard on the left side.
